This page is specifically for U.S. Armed Forces Veterans and Emergency First Responders
living with Post-Traumatic Stress Disorder AND you reside outside of Colorado

 

 

1.  Have a desire to get better

As a veteran or an emergency first responder living with PTSD, you have to want to get better. If you are not in treatment for PTSD, you are not eligible for our program. The purpose is to help you get better and that also requires treatment. A service dog is not a substitute for treatment, it’s a supplement.

2.  You MUST be in treatment for PTSD and stay in treatment throughout the entire program.

3.  Your state of residence may require you to have a "service dog in training" letter from your PTSD care provider
If so, this 
letter is to confirm that you are in treatment for your PTSD and have the need for a service dog or animal.

IF YOU ARE UNSURE WHAT YOUR STATE REQUIRES, PERFORM A SEARCH ONLINE FOR "YOUR STATE'S LAWS CONCERNING SERVICE DOGS IN TRAINING."  

 

(Search Example:  Colorado service dog in training laws)

This is extremely important for you to remain in compliance with your state's regulations.

If the letter is required by law in your state it should state:

    A.  You are in treatment for PTSD or any form of PST

    B.  You have a need for a “Service Animal” or “Service Dog”, not an “Emotional Support Animal”.

4.  Complete the HPFL Commitment Document & a copy of your DD214

You must complete the HPFL Commitment Document and provide us with a copy of your DD 214 discharge form.  

5.  HPFL cannot provide you with a puppy that will become your service dog in training and future service dog.  You will need to purchase your puppy.  Please see the list of breeds we can accept and breeds that we cannot accept for training as a service dog.  

6.  You must complete the HPFL Canine Evaluation form with the commitment form for any dog you are bringing into the program.
